Medical 6th year exemption?

I read the T-star article about our guys returning from injury and am wondering if anyone can help me understand the process for qualifying for a 6th year medical redshirt? Does that, if applied for, have a timeline for being heard? If granted, would it have to be served at ISU? As a (assumed) graduate, would he be able to transfer if he so desired, or would that be localized to Indiana St.?
